We are a husband-and-wife team of medical providers with over 12 years of medical practice who are dedicated to our patients.

Bret Jacobsen is a military trained, board-certified physician assistant who has spent over 4 years in plastic and reconstructive surgery, as well as general surgery, and has a passion for beauty and aesthetics.

Jami Jacobsen is a board-certified family nurse practitioner who believes in a patient-centered approach to medicine and aesthetics. She realized early on that treating patients both on the inside and out is imperative to helping them have the best quality of life.

					Ana was incredibly kind and patient with both my boyfriend and me. She took the time to understand our different skin needs and provided expert advice throughout the facial. I highly recommend Ana to anyone looking to get a facial, as she truly knows her stuff and creates a relaxing, personalized experience.

					Love Revive! Health and spa is a must. Just had a facial with Ana! Best facial ! My skin feels so good! The atmosphere is so relaxing as well! Jaime is the best for health. She actually listens to your needs . Other doctors rush you out and want to force pills on you. Jaime gets to the actually issue you’re having . Highly recommend!

					I've been to both sides of the Revive Health and Wellness practice and receive the best care and service. Jami and Bret are fantastic human beings so why wouldn't they hire amazing people. It's refreshing to go to an office and be greeted by happy, helpful people like Diane and Pamela. My review today is for Ana the aesthetician. Today was my second facial with Ana. The way I explained it to my friend, "it's like a high-end spa experience, in little old CG."  Ana is such a sweetheart and truly cares about her clients and my skin feels AMAZING!

					Let me  start by saying Jami and Bret Jacobson are the best providers I have ever been to!!  Words cannot express, how truly grateful I am to them.  They go above and beyond for their patients, and truly CARE. Their office Staff (Diane, Kaylynn, Pamela and Ana) are absolutely the BEST and also provide excellent care and customer service!!  Thank you Bret, Jami and Staff - you really make a difference in the world❤️.

					I love this place. Both Brett and Jami are amazing. Recently been seeing Jami. She has been awesome. Very thorough with my blood work and addressing all of my concerns. Just scheduled my Hormone Replacement Therapy so I'm excited to start this journey. So happy I stumbled onto Revive!!!!

					This is a wonderful little place, the staff has A1 customer service and Jamie is a awsome nurse practitioner I am happy to be going here now.  Also have had a couple of hydration IVs, great atmosphere and you feel very welcome and not rushed. Would definitely recommend.

					Very polite, friendly, punctual and flexible if emergency arises. Very clean and up to date facility. Responds promptly with any questions, concerns or general information requested. I cannot say enough about this facility. So amazing to have a family doctor that is really there to talk to you in plain English.

					I love Revive!!! Bret and Jami are absolutely the best in all things medical! I've lost 25lbs using Wegovy with Revive!!! I get my Botox, my lunch hour facelift and my lips done! Yep I'm that chick and I'm gonna age gracefully thanks to Bret and Jami!!
